Mechanism of action

How it works

Tadalafil inhibits the enzyme phosphodiesterase type 5 (PDE5). PDE5 breaks down cyclic guanosine monophosphate (cGMP), a signaling molecule that relaxes smooth muscle in the corpus cavernosum. During sexual stimulation, nitric-oxide-mediated cGMP rises; inhibiting PDE5 prolongs its action, supporting blood flow into the penis.

Tadalafil does not itself initiate an erection — stimulation is still required. It also does not increase sexual desire.

What is known about the class

Tadalafil has a plasma half-life of approximately 17.5 hours — considerably longer than other PDE5 inhibitors — which supports both daily-low-dose and as-needed prescribing patterns.

The class must not be combined with organic nitrates: the interaction can cause a dangerous drop in blood pressure. Caution also applies with alpha-blockers and in certain cardiovascular conditions.
